WE ARE ACCEPTING NEW PATIENTS!

If you would like to set up an appointment, please call our clinic at (757) 925-2011.  

When scheduling your new patient appointment, we will collect a deposit of $60 to secure your appointment time. This deposit will be applied as a credit on your account and will go towards your first visit. Please see our cancellation policy for additional information.

If you have already set up an appointment, please complete the forms below to expedite your appointment.  If you would like, you may email these forms back to us at info@tidewateranimal.com.  


We kindly ask that for your first visit, you please arrive 30 minutes prior to your appointment time.

Please provide your pet's prior medical records before your appointment.  If you would like, we can contact any previous veterinarian's to obtain those records.

Late Policy:  If you are going to be late to your appointment, please call and let us know prior to your appointment time.  If you are going to be more than 10 minutes late to your appointment, we will need to reschedule it for a later date.  We appreciate your consideration and understanding.
